<p></a> &ldquo;I really wanted to go to college, and dental hygiene was a way for me to go. The fact that I&rsquo;m still doing this 35 years later is pretty amazing,&rdquo; Donna Ruley, RDH, says with a smile. Years ago,&nbsp; she wanted to be a teacher, but in the 1970s that wasn&rsquo;t a &ldquo;graduate and go&rdquo; kind of option. Her father wanted her to go to college and be able to start working right away, so a neighbor suggested dental hygiene. &ldquo;Now I get to teach people every day,&rdquo; she says. &ldquo;The majority of each appointment is spent on patient education.&rdquo;</p>
<p><strong>Mentoring Words of Advice</strong></p>

<li>Partner up Don&rsquo;t just choose an employer, find a dentist who appreciates your profession.</li>
<li>Don&rsquo;t sit still Surround yourself with people who push you to be better.</li>

<h2>Still Scaling</h2>
<p>If Ms. Ruley had to point to one factor that has directly influenced her longevity in the profession&mdash;beyond the flexible hours and decent salary&mdash;it would be her camaraderie with her employer, Dr. Charles Vittitow. &ldquo;He&rsquo;s a lot of the reason why I&rsquo;m still in dentistry,&rdquo; she explains. &ldquo;Working for him has helped me grow professionally and be the best I can be. He has high expectations and doesn&rsquo;t spare any expense in helping me and others to achieve them.&rdquo; Described as both&nbsp; driven and dedicated to quality care, what Ms. Ruley appreciates most about Dr. Vittitow is that he appreciates the dental hygiene profession. &ldquo;I feel like my opinion matters and he never questions my treatment choices,&rdquo; Ms. Ruley continues. &ldquo;When I talk to students in dental hygiene school, I tell them it&rsquo;s important to find someone who respects them and respects the profession. With Charlie, he&rsquo;s given me responsibility, shown me respect, and at the end of the day, I feel good about myself and the work that I&rsquo;ve done.&rdquo;</p>
<p>Dr. Vittitow isn&rsquo;t all praise and no action&mdash;he quite literally put his money where his mouth is when Ms. Ruley came to him and suggested the office look into purchasing a digital radiography system. While hygienist purchasing influence is obvious (and expected) when related to products Of all her lessons learned, such as prophy paste, scalers, or fluoride varnish, big-ticket items such as digital radiography are often&mdash;perhaps too often&mdash;a decision left to the dentist alone. &ldquo;I&rsquo;d always pushed for computers in the operatory, so I knew the benefits of going digital.&nbsp; At the time, I was in charge of the old radiograph equipment&mdash;which was dying&mdash;so I told him we either needed to buy a new one or make the jump to digital. He told me I was in charge,&rdquo; she remembers. &ldquo;I got to be the one who picked out the system. I called our local reps for different companies and brought them in, tested their products, and put a sensor in my own mouth. In the end, we went with DEXIS.&rdquo;</p>
<h2>Finding Community</h2>
<p>Another influence shaping Ms. Ruley&rsquo;s career is her involvement with the Kentucky Dental Hygienists&rsquo; Association (KDHA) and the American Dental Hygienists&rsquo; Association. When she first moved back to Louisville, she didn&rsquo;t know any other dental hygienists. She would go to continuing education events and sit by herself. Then, at one KDHA meeting, someone mentioned that they needed a secretary&mdash;Ms. Ruley raised her hand and has been an officer at component and state levels for more than 15 years. Today, she is the KDHA president-elect and was responsible for planning the 2007 annual session. &ldquo;This has been such a place of growth for me,&rdquo; she says. &ldquo;I&rsquo;ve learned so much about myself as a professional and as a person. I never knew I loved event planning until I organized the annual session!&rdquo; </p>
<h2>Resolutions</h2>
<p>Dr. Vittitow and KDHA are the two prongs that have helped Ms. Ruley set goals for 2008: To get a laser in the office, to increase membership in the association by mentoring students, and to work to get another hygienist on the state Board of Dentistry. &ldquo;These two things have helped me to become the hygienist I am today,&rdquo; she says, &ldquo;and in turn, I want to help my practice and my association grow as well. I want to take them to the next level.&rdquo; mh</p>

			<content:encoded><![CDATA[<p>It is said that the mouth is the window to the body, and I can tell you today that is more true than we once thought. I have been looking inside mouths since 1981, and as a dentist I can tell so much about my patients by just looking inside their oral cavity. My philosophy is simple. The health of your body is the most important aspect to your life, and it begins with what you put inside it. We as Americans are on the average very unhealthy. According to the April 25 issue in the archives of internal medicine that only 3% of adult Americans are considered healthy (based on 4 factors, 1. Non-smokers 2. Eat enough fruit and vegetables 3. Maintain a proper body weight 4. Exercise at least 5 times per week.)</p>
<p>Health care costs are rising because Americans do not take responsibility for their own health. People may be living longer, only because modern medicine is keeping them alive. The baby boomers make up the largest segment of the population and health care costs are rising so fast that it absolutely is a huge problem is our country. Doctors are not paid to prevent disease, only to treat it. The latest statistics are:</p>
<p><strong>Adults are unhealthy</strong></p>
<ul>
<li>1,500 of us die from cancer every day</li>
<li>1 in 3 women and 1 in 2 men will have cancer in his or her lifetime</li>
<li>1 in 8 American women will be diagnosed with breast cancer</li>
<li>Heart disease kills more women than breast cancer</li>
<li>1.1 Million Americas have a heart attack every year.</li>
<li>500,000 heart attack victims in America die each year</li>
<li>23% of Americans have hypertension</li>
<li>Americans spend $330 billion per year on heart disease</li>
<li>64% of U.S. adults are overweight or obese</li>
<li>Diabetes will increase by 165% over the next 50 years, with 29 million Americans diagnosed</li>
<li>33% of Americans suffer from arthritis</li>
</ul>
<p><strong>Our children are unhealthy too!</strong></p>
<ul>
<li>Caner kills more children than any other disease</li>
<li>By age 3, children have fatty deposits in their arteries</li>
<li>By age 12, 70% have developed beginning stages of hardening of the arteries</li>
<li>1 in 4 children are obese</li>
<li>Obesity has doubled in the last 20 years</li>
<li>Nearly 50% of obese adolescents remain obese as adults</li>
<li>In the last 20 years, type 2 diabetes has increased 10 fold!</li>
<li>Over 8 million children have asthma, up 232% in the last 40 years</li>
<li>7% of school age children are diagnosed as ADHD</li>
<li>Less than 7% of children and adolescents consume the recommended 2 servings of fruit and 3 servings of vegetables per day</li>
</ul>
<p>Dental cavities are the most common, chronic disease and we see that still everyday. Sugar is everywhere and Americans consume it at an alarming rate. This leads not only to dental cavities, but obesity and diabetes. 50 % of children under the age of 10 have at least one cavity and it has been calculated that more than 51 million school hours each year are lost due to dental-related illnesses.</p>
<p>60% of adults have periodontal disease and recent research shows the link between gum disease and an increase risk for premature births, heart and lung disease, strokes and poorly controlled diabetes.</p>
<p>Oral health is linked to head and neck cancer which kills one American per hour. Researchers are looking at the link between the HPV virus and oral cancer, and ways to diagnose breast cancer through saliva.</p>
<p>Diet and exercise are the most important aspects of your health. You are what you eat!!! We need between 9 and 13 fresh fruits and vegetables a day. We need to exercise 5 to 6 times a week. This has to become a way of life, I believe in prevention and prolongment of disease. I speak to my patients all day long about this. I know when I see an overweight patient that there are multifactorial issues going on. I can recommend the proper medical doctors and trainers that can help you achieve the results you want. I practice what I preach, I inform and try to become the gatekeeper for my patients to have more successful and fulfilling lives.</p>
<p>A beautiful smile is an asset to health. The more we smile, the more endorphins are released. This helps with self-esteem and increased attitudes which can lead to more success in our everyday lives. I have witnessed from my practice in dentistry people that are so ashamed to smile, they cover their mouths when they speak, men will grow a mustache to cover up their teeth. With today&#8217;s incredible technology there is no reason not to love your smile. People will spend thousands on a car that they will only drive for five years, and leave it parked all day and night, but your smile goes with you wherever you go, and a beautiful smile does change people&#8217;s lives. After I finish a cosmetic case I receive so much joy to see a patient return, with a new hair cut, shaven face, and a totally new look and attitude!!!!</p>

			<content:encoded><![CDATA[<p><font size="2">Soft drinks are everywhere, even in our children&#8217;s schools. As a society, we are consuming them in ever-increasing quantities. The result is a new and detrimental phenomenon: acid erosion is destroying the tooth enamel of entire generations- and once the protective coating of enamel on our teeth is lost or compromised, we are extremely susceptible to further erosion and decay.</font></p>
<p><font size="2">Acidic beverages, like soft drinks, sports drinks and artificially flavored ice teas, have replaced water as our daily choice of liquid replenishment. The difference? Water has a pH of 6.7, making it relatively neutral. Some of today&#8217;s most frequently consumed beverages have pH&#8217;s of fewer than 3.0. Scientifically, this is even worse than it sounds because each unit of change towards a more acidic numeric value on the pH register is an exponential change, creating a huge difference in effects. </font></p>
<p><font size="2">The table below shows the relative pH&#8217;s of some leading beverages. The lower the number, the more acidic and destructive the beverage.</font></p>

<p><font size="2">Fortunately, enamel is the hardest material that our body produces. It is far denser than bone and can endure consumption of controlled levels of dietary acids. However, drinking more than two or three servings of acidic beverages per day can cause enamel to be reduced to a detrimental level in time. In other words, the more you drink, the more you are literally dissolving away your teeth.</font></p>
<p><font size="2">Unfortunately, as a society we have embraced mass consumption of acidic beverages. People see ads stating that a soft drink has only a few calories and feel that gives license to ingest large amounts without consequences. <p><font size="2">The impact of this trend goes beyond the erosion of tooth enamel. Health professionals who treat kidney disease recognize the correlation between increased consumption of soda pop and increased numbers of patients on dialysis. </font></p>
<p><font size="2">Regardless, we have allowed the installation of pop machines in our schools, often as a way of generating funds for programs in need of revenue. Yet, by doing so, we have unknowingly yielded to the tactics of corporate entities bent on creating generations of soft drink addicts. Tough as it may seem, refusing to support school programs by refusing to purchase soft drinks would be doing your children a far greater service. Find a way to raise money for programs that is less costly to your children&#8217;s health. </font></p>

But according to research conducted by Beall Research &amp; Training of Chicago, a new smile will make you appear more intelligent, interesting, successful and wealthy to others as well.</font></span></p>
<p class="MsoNormal"><span style="FONT-SIZE: 11pt"><font size="2">Dr. Anne Beall, a social psychologist and market research professional carried out the independent study on behalf of the American Academy of Cosmetic Dentistry (AACD). Pictures of eight individuals were shown to 528 Americans, a statistically valid cross section of the population. The respondents were asked to quickly judge the eight people as to how attractive, intelligent, happy, successful in their career, friendly, interesting, kind, wealthy, popular with the opposite sex and sensitive to other people they were.</font></span></p>
<p class="MsoNormal"><span style="FONT-FAMILY: 'Times New Roman'"><font face="Arial" size="2">Two sets of photos were created, with each set showing four individuals before undergoing cosmetic dentistry and four after treatment. Half the respondents viewed set A, the other half set B. The eight subjects viewed by respondents were evenly divided by gender. Two had mild improvements through cosmetic dentistry, two had moderate improvements and four had major improvements to their smiles, to give a wide range for respondents to view. None, however, had visibly rotten teeth, missing teeth or catastrophically bad dental health in the before shots. Respondents were not told that they were looking at dentistry, but were asked to make snap judgments rating each person for the ten characteristics, on a scale of one to ten, with &ldquo;one&rdquo; being &ldquo;not at all,&rdquo; and &ldquo;ten&rdquo; being &ldquo;extremely.&rdquo;</font></span></p>

			<content:encoded><![CDATA[<p>Lumineers are a brand of porcelain veneer that are made by Den-Mat. We all have seen the marketing campaign advertising the benefits of Lumineers. BUT, are they the solution for every patient? Of course not, Lumineers or other types of thin porcelain veneers work well for some patients but can have significant drawbacks for others. Only an experienced cosmetic dentist can evaluate your personal situation and determine if you would benefit from Lumineers. </p>
<p><strong>The Good:</strong> <br />Lumineers are<span class="highlighted"> very strong, very thin, and sometimes can be placed over your teeth without drilling on them at all</span>. Who would not prefer a cosmetic procedure that did not require drilling or anesthesia? That, of course, is the marketing campaign. </p>
<p><strong>The Bad:</strong> <br />Cosmetic dentistry is not a slam-dunk. Looking good is only half the equation. Your teeth need to function properly and the gum tissue has to be healthy around your teeth. Imagine this: You go to the shoe store and want to buy these new sneakers that everybody wants. So, instead of taking off your old shoes you pick a size of sneakers that fits over your old shoes so that you do not have to remove your current ones. What would that feel like? What would that look like? This is often the scenario with Lumineers. <span class="highlighted">Unless you have very small teeth with lots of spaces around them, you are essentially making the tooth larger than it was supposed to be</span>. That just is not natural. The gum tissue gets irritated, the teeth become fatter, and cleaning your teeth becomes much more difficult. If you have to drill on the teeth to make Lumineers fit, then you are essentially placing what we call a conservative veneer, which can be made out of numerous types of materials. </p>
<p><strong>The Ugly:</strong> <br />Cosmetic dentistry is supposed to make you look and feel good. How many people like the look of teeth that are<span class="highlighted"> too fat, too opaque, and overall just unnatural looking</span>? I do not know very many myself. Just like the shoe example above, which also would look very strange, putting anything on top of natural looking teeth will look fat and fake. If your tooth is very dark, and the veneer/laminate is very thin, then it will essentially have to be extremely opaque to block out the dark color &ndash; like white out. That does not look natural! It is my personal opinion, and that of many other advanced cosmetic dentists, that to achieve a natural looking beautiful smile you have to design the smile so that the teeth are the ideal proportions to your face and fit on your teeth as if you were born with them. That is mostly impossible with Lumineers. </p>
<p>Do Lumineers have a place in cosmetic dentistry? They most certainly do. The idea behind Lumineers is wonderful, and when used appropriately they can be a great service to the patient. Unfortunately very few patients actually fit that narrow critera. <span class="highlighted">Here at The Center for Ideal Dentistry we place many types of veneers, each designed to give you the best possible smile in the most conservative way</span>. Properly designed veneers are inherently conservative, often only about half a millimeter thick. That is very thin, about the thickness of you finger nail. To achieve the most esthetic and healthy result it is necessary to adjust the tooth minimally so that the outcome looks great, feels great, and last a very long time. How does the dentist know what type of veneer to use? How does the dentist know how much tooth needs to be removed and where? How does the dentist know when he/she can use Lumineers? All these questions are a matter of experience and education!<span class="highlighted"> Nobody in Louisville has more experience or credentials for cosmetic dentistry than Dr. Vittitow and Dr. Hahn</span>. </p>

			<content:encoded><![CDATA[<p>Cosmetic dentistry is wonderful. Often a beautiful smile makeover can take10 years or more off someone&rsquo;s apparent age. Every dentist loves making their patients feel and look better. A new smile can change your entire life, why would you not want to have a beautiful, healthy smile? This is the point at which most patients ask me, &ldquo;How long will the new teeth last?&rdquo;</p>
<p>A beautiful smile is a combination of stunning looking teeth as well as highly functional teeth. As easy as this may sound, it is one of the most challenging aspects of dentistry today. Achieving perfect harmony between esthetics and function is the ultimate goal - I call this &ldquo;Ideal Dentistry&rdquo;. Even the most beautiful veneers won&rsquo;t last very long if they are not designed with this concept in mind.</p>
<p>Would you build a beautiful home on swamp land? Of course not. Then apply the same concept to your mouth, where your gums and the underlying bone support the teeth, onto which you build all your dentistry. If one of the supporting structures such as the bone or the gum tissue is not healthy, then the teeth will not be able to support anything &ndash; not even themselves! Building a beautiful smile requires more than just a few veneers&hellip;it requires a healthy, functional mouth first.</p>
<p>Now to answer the question, &ldquo;how long will the new teeth last?&rdquo;&hellip; At this point you probably know that the answer is - it depends &ndash; did you have your new veneers built on swamp land? Or, did you and your dentist make sure everything else in your mouth is able to support these beautiful veneers? Are your gums healthy, and do you know how to properly care for your teeth? Do you grind your teeth, do you clench your teeth, or do you bite your nails with them? All these factors and many more are critical in determining how long any type of dentistry will last. Ideal dentistry takes all these factors into account, creating a treatment plan that is geared specifically towards your needs and habits. Prior to any smile makeover you should be free of gum disease and decay, and have a plan to control the habits that have worn down your teeth to the point they are at now. Once you have your new beautiful smile you need to have &ldquo;engineering checks&rdquo; on a yearly basis &ndash; this is where the dentist checks the function of your mouth and makes certain you are not causing harm to your teeth. A slight adjustment to your bite could extend the life of your teeth for years. Of course, regular visits to the hygienist to remove bacteria and deposits below the gum tissue, among other essential tasks, is a must.</p>
<p>At this point most people are looking at me and saying, &ldquo;<strong>Yes, so how long will my new teeth last???</strong>&rdquo; Here are a few numbers to make it more clear: If the new teeth were build according to the principals of ideal dentistry, and you follow up regularly as directed by your dentist, then you can expect to have an average of 10-15 years out of your new teeth. In my personal opinion, this is a low national average and it can be much higher. Yet insurance companies define success quite differently: Most insurance companies will pay again for a tooth that was treated 5 or 10 years ago. If a new tooth only lasts 5 years, then there was something wrong, in my opinion. The goal of ideal dentistry is to restore every mouth for the last time. I fully expect most of the dentistry that we place to last 20 plus years, if routine engineering checks and hygiene visits are maintained.</p>
